Justification for change

It appears there may have been a miscommunication.  In accordance with OMB’s authorization to delete the old Appendix E (addressing adequacy of facilities), we revised the application and instructions to delete reference to the old Appendix E addressing facilities.  Attached is the revised version that conforms to OMB’s approval.   Apparently, we did not send you the revised application/instructions for uploading to ROCIS. 

Also attached are the credit counseling application/instructions that conform to OMB’s instructions during the review process (OMB instructed us to add the Burden Statement).
