RE: Data Elements Crosswalk of EHR Generic Clearance OMB 3145-0136

In response to your request for identification of common data questions in the monitoring tasks in the EHR Generic Clearance OMB 3145-0136, we have developed the attached matrix. Tasks instruments cover three common data categories: 1) Staff and Project Participant Characteristics 2) Project Implementation Characteristics 3) Project Outputs.

The following table is a crosswalk matrix that identifies common collection categories for ten collection instruments identified by their reference in the request of September 12, 2001. The Common Data Categories are footnoted to show examples of the questions in each category. As we discussed in our presentation to you, the respondents to these collections are quite diverse and thus the monitoring instruments are customized appropriately by program.
