Terms of Clearance
When we last obtained OMB approval in June 2010, OMB placed the following Terms of Clearance on the information collection request (ICR):
OMB is approving this collection with the understanding that SSA will report the collection as a PRA violation with their next Information Collection Budget (ICB) submission.
SSA agreed to the above Terms and reported this collection as a PRA violation in the FY 2011 Information Collection Budget.
Revision to the Collection Instrument
We are discontinuing the use of the Third-Party Contact Worksheet as we noticed our SSA field office employees were unsure when to use the Third-Party Contact Worksheet in lieu of other worksheets. Therefore, we simplified the process, requesting the field office staff use the Face-to-Face, or Telephone Worksheets in all their interviews, and merely reference use of third-party assistance, if applicable, in the remarks segment.
In addition, although we have made no formal decisions at this time, we are looking for ways to streamline our business processes and strengthen our anti-fraud tools. In the future, we may make additional changes to these worksheets and revise how the agency uses them for program integrity. If we choose to expand this program beyond its current parameters, we will obtain OMB’s approval for all revisions.
We are revising the PRA statement to reflect our current boilerplate language. The current language, which dates back to the last reprint of the form, is now outdated.
SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statement on the first page of the form.
