Organizational Profile

Provide an organizational profile of no more than one page. Include the following information:





Your organization’s mission or statement of purpose, noting the source, approving body, and date of the
official document in which it appears. You may excerpt from the statement or summarize it to convey the
essential points of the statement if the statement itself is too long to be quoted in full.
Your service area (communities and/or audiences served, including size, demographic characteristics, and
geographic area)
A brief history of your institution or organization
