You have been invited to participant in a research study involving electronic health record (EHR) technology. Participating in this study is completely voluntary.
Your participation involves completing a web survey about your experiences with and perceptions of EHRs. You can expect the web survey to take approximately 60 minutes to complete.
You will never be asked to enter your name or other identifying information. Moreover, we will NOT capture any other identifying information, such as your IP address.
