any person be subjected to a penalty for failure to comply with, a collection of information subject to the requirements of the Paperwork
Reduction Act, unless that collection of information displays a currently valid OMB Control Number.

The National Oceanic and Atmospheric Administration is the Federal agency that provides citizens,
planners, emergency managers and other decision makers with information concerning weather
forecasts, severe storm warnings, fisheries management, coastal restoration and marine commerce.
As part of its work, NOAA periodically surveys members of the public about the information and
services NOAA provides.
This survey should be filled out by an adult household member living at this address.
