This information
collection request is approved as revised by HHS. In addition to
the previous terms of clearance, which continue to apply, HHS use a
nonsubstantive change request to update the instruments to reflect
the results of cognitive testing of the questions identified during
the review process. Further, HHS will provide to OMB its evaluation
of the results of the use of differential incentive amounts at the
sites participating in this study. HHS will ensure in future
packages that all planned incentives are fully described in the
supporting statement and associated with the approval of the survey
round for which they apply.
