Clinic Director Script

INITIAL COMMUNICATION TO CLINIC DIRECTORS VIA E-MAIL:

Dear ______,

My name is _____ and I’m working in coordination with CDC and the local Health Department to conduct a health survey in this community.   Our company, SciMetrika, will be administering surveys to people in several local clinics to better understand issues related to people’s health, their health behaviors, and how HIV may affect them.  We are interested in conducting some of these surveys at your facility in the upcoming months, if this is amenable to you and your staff.

The survey would take about 45 minutes to complete and eligible participants will receive $XX as a token of our appreciation^a.  Our survey would not interfere with your typical patient care activities and we are not trying to sell anything; we just want to learn about health behaviors of people accessing care in clinics.  Our survey is secure and any information shared by participants will be kept completely secure to the extent permitted by law.  We would appreciate some collaboration from your clinic staff in our survey recruitment efforts, without adding a significant burden to their workload.  We would also offer $15 per successfully completed survey to the clinic as a token of our appreciation in working with us.  Would you be open to us conducting our health survey in your facility?

Given your approval to conduct this survey, it would be most helpful to have a contact person on your staff with whom we could discuss our survey implementation efforts in more detail. We only need 20-30 minutes of their time to explain the project and what’s involved.  I have attached a project fact sheet to provide more information about our SciMetrika Health Survey.  Thank you in advance for your time and assistance.  We look forward to your response and hope to move forward in our efforts to increase understanding of public health and HIV-related issues.
