The Affidavit of identifying witness
is submitted in conjunction with a application for a U.S. passport.
It is used by Passport Services to collect information for the
purpose of establishing the identify of the applicant.This
application is completed by the identifying witness when the
applicant is unable to establish his or her identity to the
satisfaction of a person authorized to accept passport
applications.

Changes due to decrease in
number of respondents and is based on the fact that the Department
is making better use of public records as they become more
available online. As such, we have access to more documentation to
corroborate an applicant's identity claim, reducing the need for
filling out a DS-71, which is primarily used to help an applicant
establish his/her identity.
