I work with _______________________________________ I would like to invite you to take part in a research study we are doing with the National Institute for Occupational Safety and Health. It concerns developing methods for improving the safety and health of workers in the United States. At this stage, we are asking some questions to help us develop a safety training program for immigrant workers. We would like you to participate in a survey that will last about one hour. The purpose of this interview is to learn more about some of your experiences at work. No other personal information will be asked during the interview. There are no foreseeable risks to you from participating in this project. There is no direct benefit to you; however we hope that the project will improve training for workers in general. In reimbursement for your participation you will be given $25. Will you be interested in participating in this interview?

Public reporting burden of this collection of information is estimated to average 3 minutes per response, including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information. An agency may not conduct or sponsor, and a person is not required to respond to a collection of information unless it displays a currently valid OMB control number.
