The purpose of the Narrative of Changes is to clearly indicate changes to a collection since the previous approval.

FEMA Form 010-0-12 (newly prescribed form developed)
The following are the changes to the collection:
FEMA Form numbers have been changed from FEMA Form 90-153 to FEMA Form 010-0-11. Newly prescribed form developed for Request for Continued Assistance data collection instrument (FEMA Form 010-0-12).
Supporting Statement:
Question 2 – Updated to reflect the newly prescribed form.
Question 3 – Updated to explain efforts to reduce the burden.
Question 8a, 8b, 8c – Updated to reflect FRN publication information and FEMA Form 007-0-14, formally known as FF 90-149.

Explain:
There is an increase program change to the total cost to respondents from $0 (2009) to $15,675,517.00 (2011) as a result of an increase in the mean wage and the introduction of a multiplier (1.4) to account for a fully loaded average wage rate.
There are no changes to the annual hour burden and there has been no change to the information being collected.
