Page 1, What is the Purpose of Form N-336?	What is the Purpose of Form N-336? This form is used to appeal an unfavorable decision for an individual applicant. You must file your request for a hearing within 30 calendar days after service of the decision (33 days if your decision was mailed) with the local office of the U.S. Citizenship and Immigration Services (USCIS) that made the unfavorable decision. *** Attorney or Representative If you wish, you may be represented, at no expense to the U.S. government, by an attorney or other duly authorized representative. If so, that person must submit Form G-28, Notice of Entry of Appearance as Attorney or Representative, with the request for a hearing. Form G-28 can be obtained by calling our forms line number at 1-800-870-3676, our National Customer Service Center, or from our Internet Web site at 1-800-375-5283 or visiting our Internet Web site at www.uscis.gov. *** Brief You do not need to submit a brief in support of your request, but you may submit a brief if you so choose. You may also submit a simple written statement instead of brief. You may also submit evidence. You must send your request and accompanying fee and documentation to the USCIS office that made the unfavorable decision. If you need more than 30 days, you must within the initial 30 day period and explain why in a separate letter attached to this form. USCIS may grant more time for good cause.	The Purpose of Form N-336 This form is used to request a hearing before an immigration officer on the denial of your Form N-400, Application for Naturalization. You must file your request for a hearing within 30 calendar days after service of the decision (33 days if your decision was mailed) with the U.S. Citizenship and Immigration Services (USCIS) address provided in “Where to File” in these instructions. *** Brief You may attach any additional documents or briefs to support your Form N-336 at the time of filing your Form N-336 or at the time of your Form N-336 hearing.
Page 1, Where to File?	You must file your Form N-336 with the USCIS office that made the unfavorable decision within 30 calendar days after service of the decision. The date of service is normally the date of the decision. Skyharbor Circle S Suite 100 Phoenix, AZ 85034 Form N-336 Requests From Current or Former Members of the Military, Spouses of Current Members of the Military, and Close Relatives of Deceased Members of the Military You must send all Form N-336s applications regarding naturalization applications filed under the military provisions, sections 328 or 329 of the INA, to the Nebraska Service Center (NSC) at the address below regardless of where you live and whether you are filing from within the United States or abroad. Also, if you are the spouse of a current member of the military, or are the close relative of a member of the military (see INA section 319(d)), send your request for a hearing to the NSC at the address below regardless of where you live and whether you are filing from within the United States or abroad. To download a copy of Form G-1145, including the instructions, click on the “FORMS” link on the USCIS Web site at www.uscis.gov. Filing Addresses The filing addresses provided on this form reflect the most current information as of the date this form was last printed. If you are filing Form N-336 more than 30 days after the latest edition date shown in the lower right corner, visit the USCIS Web site at www.uscis.gov before you file, and check the “FORMS” page to confirm the correct filing address and version currently in use. Check the edition date located at the lower right corner of the form. If the edition date on your Form N-336 matches the edition date listed for Form N-336 on the online “FORMS” page, your version is current. If the edition date on the online version is more recent, download a copy and use it. If you do not have Internet access, call the National Customer Service Center at 1-800-375-5283 to verify the current filing address and edition date. USCIS will reject forms with the wrong revision date and return the fee with instructions to resubmit the entire filing using the current form.

The check or money order must be drawn on a bank or other financial institution located in the United State and must be payable in U.S. currency: and 2. Make the check or money order payable to U.S. Department of Homeland Security, unless: A. If you live in Guam, make it payable to Treasurer, Guam. B. If you live in the U.S. Virgin Islands, make it payable to Commissioner of Finance of the Virgin Islands. NOTE: Spell out U.S. Department of Homeland Security; do not use the initials ‘USDHS” or “DHS.”	Use the following guidelines when you prepare your check or money order for your Form N-336 fee: 1. 83C revision request

The address changes to the N-336 are in conjunction with C4 release 8.1 (centralization project) that is currently scheduled to go live in October of 2011. The above changes will facilitate the N-336 to be received and accepted by the USCIS Intake Enhancement via contractor JPMorgan, while Military-based N-336 filers will file with the Nebraska Service Center. This revision will cut down on rejected and lost request for hearings as well as standardize and centralize the intake process.
