Dear Executive Director:
PIH has hired D&R International, a contractor, to survey all PHAs on energy conservation efforts in Public Housing. I am personally requesting your participation in this voluntary and anonymous survey. By providing this information you are contributing to HUD's energy action goals. The information you submit will be used to accurately create reports and strategies to reduce utility expenses through energy conservation and efficiency measures within public housing as required by 24 CFR Part 965, Subpart C.
The estimated burden of completing this form is ten minutes per response, including the time for reviewing instructions. This agency may not collect this information, and you are not required to complete this form, unless it displays a currently valid OMB control number.
The Federal Energy Policy Act of 2005 requires HUD to submit a report to Congress documenting the steps taken to reduce energy consumption and costs in Public Housing programs. By voluntarily completing this survey [the words, "this survey" will be a hyperlink to the survey] you will provide valuable input for this report.
