Justification
Part III, Question 1 asks medical professionals to provide clinical diagnosis, and DSM IV code, for an applicant when an applicant files for naturalization. At times, though, the medical professional provides a diagnosis that does not include a DSM IV code. Therefore, USCIS provides guidance to the medical professional when this occurs while requesting an explanation for the lack of DSM IV code in the diagnosis.
