You have been selected for your feedback as a health professional who is currently working in nutrition, physical activity or obesity within your state or territory health department. We are writing to ask for your participation in a short, one-time, Survey of Obesity Prevention Programs.

This survey is designed to gather information from select representatives of state and territory health departments regarding decision-making around selection and implementation of obesity interventions and trainings. Questions about the use of specific resources, such as the Center for Training and Research Translation (Center TRT) products, will also be included. The Centers for Disease Control and Prevention (CDC) funds the work of Center TRT and our team seeks to better understand knowledge, use and implementation of such services. Your feedback will be used to assess and improve the products of Center TRT and other resources available to support obesity work.

Your participation is critical to the success of this survey, as your contribution will ultimately impact resources available to yourself and other obesity health professionals.

Participation in this survey is voluntary.
