Hello my name is____________. We are following up with a few people who did not respond to our on-line survey. You may remember receiving an e-mail asking you to take a survey for the National Enhanced Elevation Study. I am hoping that you will be able to take about five minutes to answer a few questions. These questions are from the on-line survey and by answering them, you will help us complete final stages of data collection for this survey. At this time, I would like to let you know that you are not required to respond to any of these questions and that your responses will only be used to complete our survey process. May I begin?
