INFORMATION FORM

Survey of 21st Century Community Learning Center Programs


Thank you again for your participation in this survey. The survey will inform policy makers and funders about 21st CCLC programs and will contribute to our understanding of what makes a high quality early childhood afterschool program. It is possible your program(s) may be selected from the national study for additional follow-up data collection.


Once the results of this survey are compiled and analyzed, we will be randomly selecting approximately 30 programs for more in-depth exploration of program qualities and activities through site visits by trained master observers. During the two days of site visits, we would be conducting one-on-one interviews with leadership and staff, observing staff during classroom activities, surveying parents, and reviewing program records in order to better understand the nature of your program and its activities. For the follow-up data collection, we would contact you to schedule a site visit with two of our master observers.


This information form is to make you aware of this possibility and to ask for your permission to contact you in the future if your program is randomly selected for the second wave of data collection.
