Subject: Centers for Disease Control and Prevention (CDC) Model Performance Evaluation Program: Drug Susceptibility Testing of Mycobacterium tuberculosis (M. tuberculosis) and Non-tuberculous Mycobacteria (NTM)


Dear Participant:


The next shipment of M. tuberculosis and NTM strains will be shipped to your laboratory on Month, date and year. This shipment will consist of four cultures of M. tuberculosis, to further emphasize the public health importance of M. tuberculosis, and, if appropriate, one culture of NTM.


Some institutions require delivery of all incoming mycobacterial samples to a mailroom or central receiving area in a different location at their address. If this is the policy in your institution, please arrange for timely delivery of the cultures to your laboratory.


If you do not receive your culture shipment within 10–14 days of the shipping date, or if any cultures were damaged in transit, please contact the Program Coordinator, contact to be determined at xxx-xxx-xxxx (within the US) or xxx-xxx-xxxx (outside the US).
