ADVANCE NOTICE E-MAIL
The Division for Heart Disease and Stroke Prevention at the Centers for Disease Control and Prevention would like to invite you to participate in a voluntary, web-based survey to gather your feedback and opinions on the Sodium Reduction Awareness Toolkit and future sodium reduction products and tools. You will receive an e-mail in approximately two weeks inviting you to respond to this survey through SurveyMonkey. Responses will not be identifiable, and will only be used for program improvement.
