The form has been reduced from 4 pages to 3. It has also been simplified and is more user friendly. Part I - Extra blocks have been added for additional co-applicant names and address information. Parts II and III of old version has been deleted because needed data will now be obtained directly from HUD's PIC data system. Parts IV and V from the old version have been renumbered as Parts II and III of the new form. References to Homeownership coordinators have been removed because all positions are now designated as FSS coordinator positions.

There are no changes to the burden hours.
