Revisions to the Collection Instrument
Since the SSA-1010 no longer suits our purposes for this collection, SSA is discontinuing its use to collect information from a person, institution or organization that wants to reproduce, duplicate, or privately print any SSA application or other form prescribed by SSA. However, we will continue to accept requests in writing that provide the required information set forth in the regulation as shown on SSA’s website, http://www.socialsecurity.gov. As explained in the Supporting Statement, respondents may submit their requests either via email or mail using their company letterhead.
