B2. Describe the procedures for the collection of information.
There is no statistical methodology for stratification or sampling planned. The survey is a combination of yes or no questions and short answer questions. No degree of accuracy is required.
B3. Describe methods to maximize response rates and to deal with issues of non-response.
The survey is designed as a self-assessment so that respondent may use the information as they see fit or not at all. The survey is optional and is not required. The survey format allows for the survey to be completed electronically or by hard copy.
B4. Describe any test of procedures or methods to be taken.
Because the survey is designed to be a self-assessment and each respondent’s answers will be unique to only their project, no testing is necessary.
B5. Provide contact information for individuals consulted on the statistical aspects and of the person who will analyze information collected.

Each of the 750 respondents will be responsible for analyzing their own data.
