Justification for Change Request

This is a request to make a Nonmaterial/Nonsubstantive Change to the Questionnaire approved by OMB, 0693-0051, on August 1, 2008.

The Building Fire and Research Laboratory (BFRL) of the National Institute of Standards and Technology (NIST) planned to hand out paper copies of the questionnaire to occupants of the building as they exited the building during the Evacuation Drill being performed. As part of this process, BFRL has to coordinate with a designated person at the site of the drill. BFRL has received a request from a coordinator, Building Manager of a U.S. General Services Administration (GSA) building, requesting to email the questionnaire to the occupants of the building.

This option would allow the occupants to complete the questionnaire and return it to the attention of the Building Manager, who would mail the completed questionnaires back to the BFRL using a self-addressed stamped envelope. This request would add one (1) line to the instruction portion of the questionnaire that would apply to the specific situation for the building being evacuated.

Currently the Questionnaire states: “Please return this questionnaire using the provided self-addressed stamped envelope.”

BFRL would like change this to say “Please return the questionnaire using the self-addressed stamped envelope OR Please return this completed questionnaire to the attention of _____________ located ____________.”

BFRL is also requesting to use this option for future situations.

No other aspects of the questionnaire would change. The number of respondents or the burden hours would not change as a result of this request.
