Recently, your [office/hospital] participated in the [National Ambulatory Medical Care Survey (NAMCS)/National Hospital Ambulatory Care Survey (NHAMCS)], a key survey about health care in the United States. To help us improve this survey, the Questionnaire Design Research Laboratory at the National Center for Health Statistics is looking for volunteers to participate in brief follow-up interviews. These interviews will be used to help us to improve our data collection forms and plan for future years of the survey.
You will be asked to participate in a one hour one-on-one survey research interview and you will receive $100 for your participation.
Participation is voluntary, and you or your staff may refuse to answer any question or may stop participating at any time without penalty or loss of benefits.

All information collected as part of this study will be held in the strictest confidence according to Section 308(d) of the Public Health Service Act (42, U.S. Code, 242m(d)) and the Confidential Information Protection and Statistical Efficiency Act (Title 5 of PL 107-347). This information will be used for statistical purposes only. No patient names, social security numbers, or addresses are collected as part of this study. This study conforms to the Privacy Rule as mandated by HIPAA, because disclosure of patient data is permitted for public health purposes. The NCHS Research Ethics Review Board has approved this study.
