Advance Notice: Shipment of Samples for HIV Rapid Testing (HIV-RT) from the CDC Model Performance Evaluation Program


Purpose

The purpose of this letter is to inform you:


  1. that the MPEP is resuming shipments for HIV-RT participants,

  2. the upcoming shipment will contain six plasma samples for
    HIV rapid testing,

  3. to check the accuracy of your shipping information, and

  4. you must submit your results online; no hardcopy forms will be accepted.


The next shipment of samples for HIV Rapid Testing will contain six plasma samples from individual donors who are HIV-1 infected or uninfected.

Results:

The MPEP is now accepting result submission ONLY online. You will need to:

1. provide the MPEP with a valid email address, so you may receive your password, and

2. have access to the Internet.


Please check:
Information change form [enclosed]

On the information change form, check the accuracy of the:

  • mailing address (note: samples cannot be delivered to a P.O. Box)

  • sample shipping address (if different from above),

  • current contact person’s name,

  • contact e-mail address,

  • telephone and FAX numbers, and

  • current laboratory director's name.

No response is necessary if this information is correct.

Change required?

If changes are required, please make the appropriate changes, then


  • Scan and email the form(s) to MPEPHIV-RT@cdc.gov

  • FAX the form(s) to (404) 498-2372,

  • call 1-877-360-8502, or

  • return the form directly to the MPEP.


Deadline for changes: June 26, 2009


Internal delivery

Some institutions require delivery of all incoming biologic specimens to

  • a mailroom or

  • a central receiving area in a different location at their address.


Note: Please arrange for timely delivery of the specimens to your laboratory.


Late shipment
or damaged samples?

If you do not receive your specimen shipment within 4 days after the shipping date or if the samples are damaged, please contact the MPEP Coordinator, Leigh Vaughan: email LVaughan@cdc.gov , phone 404-498-2246;

the MPEP toll-free at 1-877-360-8502, or inform the Sample Contractor (contact information will be included with the samples).
