information -- this needs to include a signature -- as well as how
long the authorization is valid (probably not more than a

This form is used to collect data on
U.S. Air Force Academy applicants to assess their qualifications
for admission to the Air Foce Academy. The writing sample is used
to evaluate background and aptitude of commissioned
service.
