7. Medical examination When required, submit a medical examination report on the form you have obtained from USCIS.
A. Individuals applying for adjustment of status through a USCIS Service Center
i. General: If you are filing your Form I-485 with a USCIS Service Center, include your medical examination report with the application, unless you are a refugee.
ii. Refugees: If you are applying for adjustment of status one year after you were admitted as a refugee, you only need to submit a vaccination supplement with your Form I-485, not the entire medical report, unless there were medical grounds of inadmissibility that arose during the initial examination that you had overseas. |

Where Should You File Form I-485? Updated Filing Address Information The filing addresses provided on this form reflect the most current information as of the date this form was last printed. If you are filing Form I-485 more than 30 days after the latest edition date shown in the lower right corner, please visit our Web site at www.uscis.gov before you file, and check the Forms and Fees page to confirm the correct filing address and version currently in use. Check the edition date located at the lower right corner of the form. If the edition date on your Form I-485 matches the edition date listed for Form I-485 on the online Forms and fee page, your version is current. If the edition date on the online version is later, download a copy and use the online version. If you are filing Form I-131, Application for Travel Document, to enable you to travel outside the United States while your adjustment-of-status application is pending, you should indicate that you are requesting a refugee travel document. The relating Form I-131 for a Refugee Travel document is filed with the Nebraska Service Center, regardless of the Form I-485 filing location.

Form I-485 and Form I-140, Immigrant Petition for Alien Worker may not be filed concurrently at a USCIS Lockbox at this time. Refer to the filing instructions on the Form I-140 if you want to file the forms concurrently.
F. You are filing your Form I-485 based on an approved Form I-526, Immigrant Petition by Alien Entrepreneur (Part 2, Box “a” on the form). You must include a copy of the Form I-797C which shows that your Form I-526 was approved.

E-Notification:
If you are filing your Form I-485 at one of the USCIS Lockbox facilities, you may elect to receive an email and/or text message notifying you that your application has been accepted. You must complete Form G-1145, E-Notification of Application/Petition Acceptance, and clip it to the first page of your application. To download a copy of Form G-1145, including the instructions, click on the link www.uscis.gov “FORMS.”
4. If you are filing your Form I-485 as an Afghan or Iraqi Translator. If you are filing Form I-485 based on an approved Form I-360 for Afghan or Iraqi Translators, you must file your Form I-485 with the USCIS Nebraska Service Center.
