My name is Lea Ayers LaFave, and I am calling to follow up on the program evaluation of the Centers for Public Health Preparedness (CPHP) program that we recently sent to you [via link by email, or by mail].

We are interested in your experiences and satisfaction with the CPHP program and also your suggestions for improving training and technical assistance activities intended to strengthen public health preparedness.
