During the conduct of the Commission's review of this information collection, it was discovered that a typographical error was made in the calculation of the estimated burden hours for the 2005 extension request. The total annual burden hours should have been reported as 9,200 hours rather than 6,200 hours. With this submission to the OMB, we are reporting more accurate estimates.
