SSA is making the following revisions:
Explanation of Changes
SSA made minor language changes to clearly explain that benefits would be permanently reduced.
Change 1: The word “permanently” was added to the explanation before question 3.
Justification: To clearly explain that benefits would be permanently reduced
Change 2: Question 3, the word “permanently’ was added
Justification: To clearly explain that benefits would be permanently reduced
Change 3: Question 4 was deleted from form SSA-25.
Justification: On the application for spouse’s benefits, we ask the applicant is whether he or she has had active military or naval service after September 7, 1939 and before 1968. Therefore, there is no reason to repeat the question when the claimant elects reduced spouse’s benefits.
Change 4: Privacy Act Statement
Justification: SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statement on the first page of the form.
Change 5: Paperwork Reduction Act Statement
Justification: We are revising the PRA statement to reflect our current boilerplate language. The current language, which dates back to the last reprint of the form, is now outdated.
