Introduction

Good [morning/afternoon]. This is _______ from RTI International. We’re working with the CDC on a project to prevent carbon monoxide poisoning.

We’d like to discuss [your/our] work on carbon monoxide [research/prevention] and learn more about your organization’s activities. Do you have time to speak with us now? The questions will only take a few minutes.

Before I start, I just want share with you that:

• We are doing this work for the Centers for Disease Control and Prevention.

• The goal of this interview is to learn how to better address and prevent carbon monoxide poisonings.

• We are calling 3 to 4 other similar organizations to talk about the same issues.

• You participation is completely voluntary, and you don’t have to answer any questions that you do not feel like you can or should answer.
