JUSTIFICATION FOR THE NON-SUBSTANTIAL CHANGE FOR INFORMATION COLLECTION 3060-0386
The Commission received OMB emergency approval for the data elements contained on the attached form – Notifications Analog Termination or Reduction on March 4, 2009. However, three data elements were inadvertently omitted from the submission. The items that were omitted in error are under the following sections: B) Telephone Help Center(s), C) Walk-In Help Center(s) and 7) Consumer Referral Contact Number. This non-substantial change request does not have an impact on the annual burden hours or annual cost burden to the respondent.
