The two new rows required for ETA 2112, although necessitated by the American Recovery and Reinvestment Act of 2009 (ARRA) for monitoring the addition of $25 in federal funds to each state unemployment check, will be permanent additions to the form. Thus, they will be available for the comprehensive tracking of future such federal interventions as well as the current one.
Two Unemployment Insurance Policy Letters (UIPLs) pertinent to this ARRA-driven addition of the Federal Additional Compensation (FAC) Program have been uploaded as supplementary documents in the IC section of this ROCIS submission. UIPL 11-09, “New Temporary Federal Additional Compensation Program,” provides states with instructions for implementing and operating the new temporary Federal Additional Compensation program, including fiscal and reporting instructions. UIPL 12-09, “Extended Benefits Program,” advises states of temporary changes to the permanent federal-state extended benefits program.
This is a fully automated collection. The forms included are representational of the data screens used by the states and jurisdictions.
We are including ETA Handbook 402 as a supplementary document, in lieu of all web screens, because it is the overall user guide for all Unemployment Insurance Required Reports (UIRR) and includes sample screen shots.

All UIRR reporting is integrated and electronic. USDOL provides computers to each state and jurisdiction along with UIRR reporting software and a proprietary network to use to transmit data to the computers at the USDOL National Office. Handbook 402 is complete guidance for all aspects of the submittal portion of UIRR reporting. It governs how states access the system, enter, upload, save and/or transmit data to the national office. It includes example screenshots of these processes. In the appendices, it lists the data maps for each report along with the ASCII upload formats and the reporting edits (QA/QC measures) that the state and federal databases place to the data submitted.
