Addendum to the Supporting Statement for

Revision to the Collection Instrument:

1. Removal of Code of Federal Regulation Sections Covered Under Approved OMB Control Numbers:

SSA is removing the following sections from the ICR as they are now collected under OMB approved OMB Control Numbers. The below chart includes both the Part 405 citations as well as comparable citations from Parts 404 and 416 of the Code of Federal Regulations, as well as citations from the US Code which OMB approved and the Control Numbers under which they are approved:

2. Removal of Code of Federal Regulation Sections Not Covered Under Approved OMB Control Numbers:

• SSA is removing the following regulation citations, as there were no more respondents as of the end of the calendar year 2008 due to the discontinuation of the agency initiative that utilized these laws:

• 404.1519

• 416.919m

• SSA is removing the following regulation citation, as our current data indicates there are fewer than 10 annual respondents:

• 405.427(c)

3. Additions to the ICR:

SSA is adding comparable Code of Federal Regulation sections from Parts 404 and 416 as appropriate within the clearance request. This will ensure that OMB approves all related reporting requirements.

SSA is also attaching a document explaining the burden requirements requested through these regulations. There are no standard forms for the collection of this information.
