Summary of Changes on the Annual Progress Report for the STOP Formula Grant Program for the STOP state Administrators Form

Question 6A will have drop downs for five years (five open awards).

Another column was added to 6A entitled " Percentage of Total Returned".

Question 6B now asks if any of the FFY allocation was returned unused by subgrantees and another line was added after "Victim Services" for "Discretionary”.

Question 7 adds a line for "Administrative Costs" after "Discretionary" in the chart for a five year period. A third column (in addition to Number and Amount) was added called "Percentage of Total Amount Awarded to Subgrantees During Current Reporting Period

In question 7A, the larger textbox for use of discretionary funds was added.

In question 7B, the parenthetical instruction was changed to read: If you reported administrative costs in question 7, please provide a detailed description of the amount and description of administrative costs. Please include all personnel costs, membership dues, association fees, training, travel costs for subgrantee monitoring, etc" and a larger narrative box was added.

Question 8A has an additional column for "Total Percentage of Victim Service Allocation" for 5 different fiscal years. Question 8B has a narrative box with an instruction "If your culturally specific victim service awards do not represent 10% of your total victim service allocation, please explain why and detail what efforts are being made to increase representation of culturally specific victim service providers in your subgrantee population."

In question 13 asks "Please include any areas where you might wish to receive OVW-funded technical assistance."
