Dear Dr. «LastName»:
About two weeks ago we sent you, via FedEx, a survey focused on physicians’ knowledge, attitudes, and practices with respect to post-treatment follow-up care of adult cancer survivors. A check for $50 was enclosed in the survey package in appreciation for your participation.
As an office-based practitioner, you play a critical role in providing care to cancer survivors. There are over 10 million survivors of cancer in the United States, yet little is known about how and where they are treated after the acute phase of cancer treatment. We are interested in your views of how these individuals are treated in your practice, and how treatment in the future might best be implemented. Currently, there are no nationally representative data available in this subject area.
If you have already completed and returned the questionnaire, thank you. If you have not yet done so, I hope you will take a few minutes to complete and return the enclosed survey at your earliest convenience. If you would prefer to provide your responses by telephone rather than fill the form out by hand, please call to set up a time at 800-XXX-XXXX Ext XXXX, weekdays between 9 a.m. and 5 p.m. Eastern time; or e-mail ResearchAsst@westat.com. If you have more general questions about the study, please call Dr. Contact Person at NCI (301-XXX-XXXX or contactperson@mail.nih.gov), or Dr. Caroline McLeod, Westat, (800-937-8281 ext. 2786 or carolinemcleod@westat.com).
Your participation in this survey is both critical and much appreciated. Thank you in advance for your valuable contribution to this important study.
Sincerely,
Dr. Noreen M. Aziz Dr. Paul K. Han

Dear Dr. «LastName»:
Some time ago we sent you, via FedEx, a survey focused on physicians’ knowledge, attitudes, and practices with respect to post-treatment follow-up care of adult cancer survivors. We are contacting you today because we have not received your completed survey and responses from physicians like yourself would ensure the data obtained from this study is truly representative of the physician population.
As an office-based practitioner, you play a critical role in providing care to cancer survivors. We need your views in order to understand what care is being provided, and what barriers exist to providing follow-up services related to cancer survivors. Currently, there are no nationally representative data available in this subject area.
We understand your time is limited; however, we would very much appreciate you taking a few minutes to complete and return the enclosed survey at your earliest convenience.
Sincerely,
Caroline McLeod, Ph.D.
