SALE PERMIT FOR ALASKA REINDEER

________________________________, Permittee, whose address is _______________________ __________________________________________ is authorized to purchase up to ________ live reindeer from _______________________________ of ____________________, Alaska for the purpose of butchering them within 30 days, or shipping them live out of Alaska. This permit is valid for ninety (90) days from the above date.
Chapter 897, Public Law No. 413 of the Seventy-fifth Congress, Sec. 10, codified as 25 U.S.C. 500 et seq., authorizes the sale of Alaskan reindeer to non-Native buyers only under the following conditions: Each reindeer sold shall either be butchered in the State of Alaska within thirty (30) days of the date of transfer to Permittee, or if not butchered, shall be shipped alive out of Alaska within thirty (30) days of such date, and they and all their descendants shall never be brought back to Alaska alive. This sale and shipment must also comply with all relevant State and Federal animal health laws and regulations. Failure to comply with the conditions of this Permit shall void any title to, or right to possession of, or any further purchase of, Alaskan reindeer. Permittee agrees to provide the information required on the bottom part of this form, and to mail the completed form to the permit-issuing officer within thirty (30) days of the date of transfer.

Paperwork Reduction Act Statement: This information is collected to manage the reindeer program in Alaska and to monitor their use by non-Natives.. The information is supplied by a respondent to obtain or retain a benefit, that is, obtaining reindeer in Alaska. It is estimated that responding to the request will take an average of 10 minutes to complete. This includes the amount of time it takes to gather the information and fill out the form. If you wish to make comments on the form, please send them to the Information Collection Clearance Officer, DAS-IRM/CIO, 625 Herndon Parkway, Herndon, VA 20170. Note: comments, names and addresses of commenters are available for public review during regular business hours. If you wish us to withhold this information, you must state this prominently at the beginning of your comment. We will honor your request to the extent allowable by law. Privacy Act Statement: This information collection document contains information that is covered under the Privacy Act, as amended. Collection of this information is authorized under 25 U.S.C. 500.
