ADDENDUM TO THE SUPPORTING STATEMENT

FOR formS SSA-3192, SSA-3193, AND SSA-3194

We are making the following changes to this information collection:

Change 1:

Previously, we had one only form under this OMB number, which was completed by the SSA employees’ physicians. In the current ICR, we now have three different forms – an initial application completed by SSA contractors, a verification form completed by the physician, and a follow-up form the medical parking permit holders complete to verify their continued need for the permit.

Justification for Change 1:

Respondents found our original one-form method to be somewhat confusing. We believe this method will allow for greater clarity, and will enable physicians to more easily complete and assemble the necessary information.

Change 2:

Previously, when the physician retrieved the form, the applicant had to retrieve it and submit it to SSA. Form SSA-3192 now instructs physicians to submit the form and accompanying evidentiary documents directly to SSA.

Justification for Change 2:

This new methodology will save time and will allow the direct transfer of medical documents from the doctor’s office to SSA.
