12. Estimate of the hour burden of the collection of information:
It is estimated that it will take 10 minutes for respondents to respond to the survey. A total not to exceed 1,000 surveys will be mailed out with an expected 40% to be returned (400). The number of respondents = 400 x 10 minutes to respond to survey = 4,000 burden minutes, divided by 60 minutes = 66 hours with each iteration estimated to occur annually. It is estimated that there will be two focus groups at 4 facilities to occur annually. Each focus group will be one hour in length and 10 participates per focus group, given a total of 80 participates. The number of participants = 80 x 60 minutes, divided by 60 to respond to focus group questions = 80 burden hours. The total burden hours for the surveys and focus group would be 146.
