Acknowledgement of Participation
By answering the web survey, I am volunteering to take part in a study on how people obtain government information from libraries and public access computing centers that is conducted by the Library Research Center, of the University of Illinois at Urbana-Champaign, in cooperation with the Institute of Museum and Library Services.
This survey will take no more than ten minutes of my time.
By participating in this research, I am contributing to society by helping libraries and public access computing centers to meet the information needs of the public better.
By completing the web survey and electronically submitting my answers, I give permission for the Library Research Center to use my responses to prepare a summary report, with the understanding that there will be no identifying information including in the report.
Further, I understand:
I can skip any survey question with no penalty.
I can opt out of this study at any point with no penalty.
The reports prepared for this study will summarize findings across the sample and will not associate responses with a specific [institution or] individual. The researchers will not provide information that identifies me [or my institution] to anyone outside the study team, except as required by law.
There are no risks beyond those in everyday life associated with my participation in this research.
