Minor Revisions to the Collection Instrument

SSA is making two minor revisions to the SSA-702:

• We are correcting a typographical error in the instruction under the first Name blank on the form where it states “(Name of wage earner, self-employer person, or SSI applicant).” The correction will read as follows: (Name of wage earner, self-employed person, or SSI applicant).

• As per the instructions from the SSA Office of the General Counsel, we are adding a Privacy Act Statement to the second page of the form. Previously we did not have a Privacy Act Statement.
