Updates to Pilot

REVISIONS TO SUNWISE STUDENT SURVEY

Based on the qualitative interview results and the quantitative IRT analysis results, the following changes to the SunWise student survey were made.

• Instructions

  • Added instructions to “Please Turn Over” at the bottom of the first page, to the left of the SunWise logo.

• Question 11

  • Made the instructions regarding selecting multiple options more visible by increasing the font size.

  • Changed the Question 11 stem to: “Some of the reasons why I do NOT always wear sunscreen when I’m outside are because:”

  • Changed the Question 11 last answer choice to: “None – I always wear sunscreen!”

  • Added the answer choice: “Sunscreen feels greasy on my skin.”

• Question 12

  • Made the instructions regarding selecting multiple options more visible by increasing the font size.

  • Changed the Question 12 stem to: “Some of the reasons why I do NOT always wear a hat when I’m outside are because:”

  • Changed the Question 12 last answer choice to: “None – I always wear a hat!”

• Question 15

  • Made the instructions regarding selecting multiple options more visible by increasing the font size.

  • Added answer choice “My neck”

• Question 16-17

  • Combined the two questions and change question stem to: “Have you ever reminded a:”

  • Changed secondary question stems to:

    • “Friend to put on sunscreen?”

    • “Sibling (brother or sister) to put on sunscreen?”

    • “Parent or guardian to put on sunscreen?”

    • “Parent or guardian to put sunscreen on you?”

  • Kept all responses yes/no.

• Questions 18-21

  • Replaced three answer choices with a five-point scale “Never,” “Rarely,” Sometimes,” “Often,” and “Always,” if space permits.

• Questions 22-24

  • Moved questions into the breakout box “When you were outside in the sun last summer”

• Question 26

  • Changed “when you go out in the sun” to “when you go outside during the day.”

• Questions 29-31

  • Deleted Question 31.

  • Moved questions into the breakout box “When you were outside in the sun last summer” and moved this box to the end of the survey.

  • For Question 29, changed question stem to: “Last summer, how many days a week did you spend outside during the day from 10AM to 4PM?”

  • For Question 30, changed question stem to: “On the days that you went outside last summer, how many hours a day did you spend outside from 10AM to 4PM?”
