This is a request for OMB to approve the Short Follow-Up Questionnaire 2008 in the
prospective cohort study of diet and cancer in members of the American Association for
Retired Persons (AARP) OMB# 0925-0423, expiration 12/31/2005. A completed Part 1
Worksheet, Federal Register Notices, supporting statements, and various attachments
accompany this memorandum.
The purpose of the NIH-AARP Diet and Health Study Short Follow-Up Questionnaire
2008 is to continue to examine prospectively the relationship between diet and major
cancers in early to late-middle aged men and women in the United States by obtaining
information on selected medical conditions, medical procedures, and physical status and
lifestyle questions. Additionally, another objective of the Short Follow-Up Questionnaire
2008 is to maintain active follow-up with our participants. Collection of this information
will allow the ongoing examination of health and lifestyle behaviors and contact with our
study participants.
A total of 513,225 live, eligible participants will be asked to complete the Short FollowUp Questionnaire 2008. The annualized burden for the completion of the questionnaire
by all respondents is 34,283 hours.
A mission of the Division of Cancer Epidemiology and Genetics at the NCI is to establish
and support programs for the prevention and treatment of cancer. The NIH-AARP Diet
and Health Study Short Follow-Up Questionnaire 2008 is an important instrument that
supports our ability to further examine the relationship between nutritional exposures and
cancer.
This request is for the Short Follow-Up Questionnaire 2008. The NIH-AARP Diet and
Health Study previously received OMB approval in 1995 (OMB# 0925-0423).
