DDI Patient Survey – Frequently Asked Questions (FAQs)

What is the purpose of this study?

We are doing this survey to assess possible reasons why people choose to be screened for diabetes. Some factors might include characteristics of the screening program, patient background as well as the cost of health care. This information will provide the Centers for Disease Control with some of the answers they need in planning and evaluating diabetes screening programs in this and other communities.

Why am I being asked to participate?

We are asking people age 40 and older who have not been previously diagnosed with diabetes to participate in this survey. We hope to talk with 600 clinic patients in 10 regions of the country at clinics that participated in pilot test of the DDI. The Diabetes Detection Initiative (DDI) was a pilot program with the goal of screening and detecting people with previously undiagnosed diabetes. It was implemented during late fall 2003 through spring 2004. Your participation in this survey will provide us with background information about yourself, information on the costs associated with your clinic visit, and the features you prefer in a diabetes screening program. This survey should take about 20 minutes to complete and can be done while you wait to see your health care provider today.

Are my answers to the survey confidential?

We never link survey answers to your name. We keep the signed documents that contain your name, separate from your completed survey and in a secure location so that no one outside of the research staff will know you participated. When you type your answers into the computer, they are coded with a unique identifier that cannot be linked to your name. Your survey answers are only used for research purposes and will be presented in a summarized form. Your answers cannot be linked back to your name and no individuals are identified in summaries or reports.
