VA Form 21-0788 is used to gather
information from veterans and claimants that is necessary to
determine whether benefits may be apportioned to the veteran's
spouse, child, or dependent parent, and, if so, the amount that may
be reasonably apportioned. Without this collection of information,
VA would be unable to properly authorize apportionments of
compensation and pension benefits.
