Review; Comment Request
AGENCY: Federal Emergency Management Agency, DHS.
ACTION: Notice; 30-day notice and request for comments; Revision of a
currently approved collection, OMB Number 1660-0004, FEMA Form 81-64.
----------------------------------------------------------------------SUMMARY: The Federal Emergency Management Agency (FEMA) has submitted
the following information collection to the Office of Management and
Budget (OMB) for review and clearance in accordance with the
requirements of the Paperwork Reduction Act of 1995. The submission
describes the nature of the information collection, the categories of
respondents, the estimated burden (i.e., the time, effort and
resources
used by respondents to respond) and cost, and includes the actual data
collection instruments FEMA will use.
Title: Application for Participation in the National Flood
Insurance Program.
OMB Number: 1660-0004.
Abstract: The National Flood Insurance Program (NFIP) provides
flood insurance to communities that apply for participation and make a
commitment to adopt and enforce land use control measures that are
designed to protect development from future flood damages. The
application form will enable FEMA to continue to rapidly process new
community applications and to thereby more quickly provide flood

insurance protection to the residents of the communities.
Participation
in the NFIP is mandatory in order for flood related presidentiallydeclared communities to receive Federal disaster assistance.
Affected Public: Federal, State, local, or Tribal Governments.
Number of Respondents: 187.
Estimated Time per Respondent: 4 hours.
Estimated Total Annual Burden Hours: 748 hours.
Frequency of Response: On Occasion.
Comments: Interested persons are invited to submit written
comments
on the proposed information collection to the Office of Information
and
Regulatory Affairs, Office of Management and Budget, Attention: Nathan
Lesser, Desk Officer, Department of Homeland Security/FEMA, and sent

395-6974. Comments must be submitted on or before January 7, 2008.
