Attached are the Internet instructions for completing the SSA-827 (Authorization to Disclose Information to the Social Security Administration (SSA)) located on our website at www.ssa.gov.

We have adjusted the burden hours to reflect the added time for those claimants who use the Internet to read the explanation screens about the form.

For claimants who do not use the Internet, our field office claims representatives explain the use and completion of the form during the disability interview that they conduct with claimants.
