his/her death.

Let me explain why I am telephoning. Heart
disease has been so widespread in the United States that the
National Institutes of Health is conducting a study to look
at its causes and treatment. I understand from our records
that <name of decedent>, a participant in this study, died in
(month and year). I would like to talk to you about him/her.
<name of decedent> gave us his/her consent
to contact a family relative if we needed additional medical
information. The events surrounding <name of decedent>'s
death may provide clues which will help in the fight against
heart disease. Would you be willing to answer a short questionnaire?
