AGREEMENT AND RECORD OF THE DISPOSITION OF FSA SECURITY/RELEASE OF PROCEEDS
This form is used to provide the expected source of the agency’s payment and to plan release of proceeds. The agency, in consultation with the borrower, completes the form. After its completion, the borrower and the agency sign it at the County Office.
