Advance Notice of Shipment of Samples for HIV-1 Antibody Testing from the CDC Model Performance Evaluation Program

Purpose

The purpose of this letter is to give you advance notice of the shipment of six plasma samples for HIV-1 antibody testing and to check the accuracy of your shipping information. The next shipment of samples for HIV-1 antibody testing will contain six plasma samples from individual donors who are HIV-1 infected or uninfected.

Sample Shipment date

January 23, 2007

Check information change form enclosed

On the information change form, please check the accuracy of the shipping address, mailing address, contact e-mail address, telephone and FAX numbers, and current laboratory director's name. No response is necessary if this information is correct.

Change required?

If changes are required, please make the appropriate changes, then FAX the form to (404) 325-2667, or call 1-800-642-6941, or return the form directly to Constella Group, LLC in the enclosed pre addressed envelope. Deadline for changes: December 22, 2006

Internal delivery

Some institutions require delivery of all incoming biologic specimens to a mailroom or a central receiving area in a different location at their address. Note: Please arrange internally for timely delivery of the specimens to your laboratory.

Late shipment Or damaged samples?

If you do not receive your specimen shipment within 4 days after the shipping date or if the samples are damaged, please contact the MPEP Coordinator, Zeptometrix at (508) 520-0588. Deadline for requesting a second shipment: February 5, 2007
